Wow, finally a game I’ll play that’s using Avatar Awards. (The other two currently with awards are Splosion Man, which I wasn’t going to pay for though the demo was fun, and Lips Number 1 Hits. . . which is more Kreyg’s thing than mine.)
That is, assuming that Fade Guy from the Steam forums is to be trusted. Mr. Guy apparently dug through the Left 4 Dead 2 demo files and found a list of Avatar awards and their appropriate requirements:
Med Kit: Earned by beating all five campaigns.
Bull Shifters Shirt: earned by winning 10 games of Versus.
A Left 4 Dead 2 shirt: Earned by winning 10 games of Scavenge.
Zombie Hand Shirt: Earned by killing 10,000 Infected.
Depeche Mode Shirt: Earned by rescuing Gnome Chompski from the Dark Carnival.
Here’s the actual text he found, which (to me) indicates that there are a few more. . . I’m not sure what “In Game” means versus “XLAST”, however.

According to Destructoid, G4 has confirmed that there are Avatar Awards in Left 4 Dead 2.

Avatar awards are better than achievements in my opinion because they actually give you something besides a number score that nobody cares about.
